CONSENT TO TELEHEALTH SERVICES

Introduction

This Consent to Telehealth Services ("Consent") explains the nature of telehealth services available through Revive Longevity LLC ("Revive Longevity") and our affiliated clinical partner, Telegra MD, LLC ("Telegra MD"). Please read this Consent carefully before using our services.

Revive Longevity is a health and wellness platform that facilitates access to telehealth services. Revive Longevity does not practice medicine, employ physicians, or provide clinical care. All clinical services, including consultations, diagnoses, treatment recommendations, and prescribing, are provided by independent licensed healthcare professionals through Telegra MD and its affiliated professional entities.

What Is Telehealth?

Telehealth (also called telemedicine) involves the use of electronic communications, information technology, or other means to enable a healthcare provider and a patient at different locations to communicate and share health information for the purpose of providing healthcare services. Telehealth may be used for assessment, treatment, diagnosis, prescribing, follow-up, and patient education.

The types of electronic transmissions that may occur include: exchange and review of medical information (health records, images, diagnostic results) between you and a provider via asynchronous or synchronous communications; two-way interactive audio; two-way interactive audio and video; treatment recommendations based on clinical review; delivery of consultation reports with diagnoses, treatment plans, or prescriptions; and prescription refill reminders.

Benefits of Telehealth

Telehealth provides several benefits, including: improved access to healthcare providers by allowing you to consult with a provider from your home, office, or other convenient location; improved efficiency in accessing medical evaluation and management; reduced wait times compared to traditional in-person visits; and increased convenience for follow-up appointments and ongoing care management.

Risks and Limitations of Telehealth

As with any form of healthcare, there are potential risks associated with telehealth, including but not limited to:

Technical problems: Issues with video quality, sound quality, or internet connectivity that may require an encounter to be rescheduled.

Privacy: Although security measures are in place, there is a possibility that security protocols could fail, resulting in a breach of privacy of personal medical information.

Limitations of remote care: The provider cannot conduct a hands-on physical examination and must rely on the information you provide and what can be observed through available technology.

Clinical limitations: In some cases, the provider may determine that telehealth is not appropriate for your condition and may recommend an in-person evaluation.

By consenting to telehealth services, you acknowledge and understand the following:

You have read this Consent carefully and understand that using telehealth may provide substantial benefits, but, as with any medical service offered, results cannot be assured, and there are potential risks that may be involved in the use of telehealth services for which Revive Longevity shall have no liability.

There are potential risks associated with the use of telehealth services, including, without limitation, that the information transmitted may be less comprehensive than that available during an in-person visit, which may affect the accuracy of diagnosis or medical decision-making; delays in medical evaluation or treatment could occur due to deficiencies or failures of the telehealth equipment; security protocols could fail, causing a breach of privacy; and/or miscommunication may occur due to technology issues, and certain diagnostic tests may not be possible to perform remotely.

Telehealth often involves the electronic transmission of your protected health information (PHI). Your PHI includes, without limitation, your individually identifiable health information; medical history; diagnoses; and communications to and from your healthcare providers. You understand that PHI may be lost due to technical failures, cyber intrusion, or other issues disrupting your telehealth visit or causing delays in response from your provider. You assume all such risks and hold Revive Longevity, Telegra MD, and their respective affiliates, officers, directors, and agents harmless from any and all claims arising out of or related to the use of telehealth services, including but not limited to any loss or breach of PHI. You understand that PHI obtained during the telehealth visit will not be disclosed to others without your consent unless permitted by applicable law and in accordance with Revive Longevity’s Privacy Policy.

You understand the alternatives to telehealth, such as an in-person encounter, as they have been explained, and in choosing to participate in a telehealth visit, you understand that some parts of the exam may require in-person physical testing to be performed at the direction of your healthcare provider.

If you are experiencing a medical emergency, you should dial 911 immediately. Telehealth services are not appropriate for medical emergencies.

You have the right to withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment.

All existing confidentiality protections under applicable federal and state laws apply to your telehealth interactions.

The provider’s clinical decisions are based on the information you provide. You are responsible for providing complete, accurate, and truthful information about your medical history, current conditions, and medications. You must keep your healthcare provider informed of any changes in your health status, diagnoses, symptoms, treatments, and medications.

The provider may not have access to your complete medical record. The quality of clinical recommendations depends on the accuracy and completeness of the information you provide.

No guarantees are made regarding the results of any treatment. A healthcare provider will provide care consistent with prevailing medical standards but cannot assure a specific outcome.

Failure to follow treatment recommendations and guidelines will affect treatment outcomes, and you must inform your healthcare provider immediately of any adverse effects of treatment. YOU UNDERSTAND THAT IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, YOU MUST CALL 911.

Telehealth services are not a replacement for your primary care physician or annual check-ups. You should continue to seek routine in-person medical care as appropriate.

At any time a healthcare provider made available through the platform may determine that your condition is not appropriate for telehealth services and you may be asked to seek care with your primary care physician or some other provider.

Neither Revive Longevity nor Telegra MD operates as an online pharmacy. Prescriptions are fulfilled by licensed partner pharmacies.

Any prescription obtained through the platform will be used only for its intended medical purpose.

Neither Revive Longevity nor Telegra MD guarantees that a specific medication will be prescribed.

The laws of the state in which you are physically located at the time of service will apply to your receipt of telehealth services.

You have the right to review and receive copies of your medical records, subject to applicable law and standard policies.

You have been advised of all potential risks, consequences, and benefits of telehealth, including risks related to the security of electronic communications. You have had the opportunity to ask questions about the information presented within this Consent by contacting Revive Longevity prior to proceeding, and you understand the information contained herein.

Consent to Communications

By providing your email address or phone number, you consent to receiving healthcare-related communications at the contact information you have provided. These communications may include follow-up instructions, educational information, prescription and fulfillment updates, appointment reminders, and other care-related messages. You may opt out of non-essential communications at any time. Standard text messaging rates or cellular phone charges may apply as provided in your wireless plan.

Recording

Telehealth sessions may be recorded for quality assurance, operational improvement, training, and patient safety purposes. By proceeding with a telehealth session, you expressly consent to such recording.

Duration of Consent

This Consent will remain in effect for all telehealth sessions and services you receive through the Revive Longevity platform until you revoke it in writing by sending written notice to support@revivelongevity.com or the mailing address listed in this Consent. Revocation of consent will not affect services already rendered.

Your Consent

By using the Revive Longevity platform and proceeding with a telehealth consultation, you confirm that you have read and understand this Consent, that you understand the risks and benefits of telehealth, and that you consent to receiving healthcare services via telehealth.
