Tirzepatide is a single-molecule dual agonist; it binds and activates both the GLP-1 and GIP receptors simultaneously. GLP-1 slows gastric emptying and signals fullness to the brain, while GIP plays a complementary role in how the body processes fat and glucose. Activating both pathways is what distinguishes tirzepatide from single-action GLP-1 medications such as semaglutide.
In clinical studies of the FDA-approved tirzepatide products (Mounjaro® and Zepbound®), patients experienced significant reductions in body weight and improvements in metabolic markers when the medication was combined with diet and physical activity. Compounded tirzepatide contains the same active peptide, prepared by a state-licensed compounding pharmacy. Warning: Risk of Thyroid C-Cell Tumors
Tirzepatide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors in rats. It is unknown whether tirzepatide causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans. Tirzepatide is contraindicated in patients with a personal or family history of MTC and in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Common side effects may include: nausea, vomiting, diarrhea, decreased appetite, constipation, abdominal pain, indigestion, injection-site reactions, fatigue, and hypoglycemia (especially when used with insulin or sulfonylureas).
Serious but less common risks may include: pancreatitis, gallbladder disease, severe stomach problems, acute kidney injury, severe hypoglycemia, diabetic retinopathy complications, and severe allergic reactions.
